Total fee is $130 - Hicaps provided onsite. This includes a Chiropractic, Orthopedic and neurological assessment as well as the report of findings on a subsequent visit.
Please note this does not include any treatment- this will be a subsequent fee of between $62-$65

New Pregnant Chiropractic Patient - $50.00

New Patient DVA - $2.00

For new patients only

This is for Department of Veteran Affairs clients only. A GP referral is required and must be bought to the initial appointment or emailed prior as well as your physical White or Gold Card.
Referral must be addressed to Dr Madeleine Yeates and have your DVA approved diagnosis on it.
A deposit is required but will be refunded after the competition of your initial consultation with Dr Yeates.

New Patient EPC Referral - $50.00

For new patients only

This is for new clients to the practice who have an EPC referral from your GP.
We also require a $50 deposit to secure your appointment, which will be deducted on the day from your total. Please note we have a 12 hour cancellation policy, we can be reached via email or on 08 9505 8480. Cancellations within 12 hours or late reschedules will result in a loss of the $50 deposit.
You will need to bring your EPC referral and Medicare card on the day. Please note your EPC covers only a portion of the visit and you will have to pay a gap fee of $69.55 on the day less your deposit.
Please note this does not include any treatment- this will be a subsequent fee of between $62-$65
Subsequent visit gap payment is from $1.55

Inactive Patient Reactivate Adjustment 6month - $30.00

For exciting Chiropractic patients of Baldivis Wellness Centre who have not been to the practice in 12 months or longer.
This is a longer consultation where the Chiropractor will conduct an updated assessment. Adjustment included.
$90 with the $30 booking fee deducted + private health deductions

IC+ Acupuncture/Cupping - $50.00

For new patients only

Acupuncture is a traditional Chinese medicine technique that involves inserting very fine needles into specific points on the body to stimulate natural healing processes, relieve pain, reduce stress, and restore balance to the body’s energy flow (Qi). It is commonly used for conditions such as chronic pain, anxiety, headaches, and digestive issues. Total consultation fee is $140 HICAPS and afterpay onsite. Please note we have a $50 deposit which is nonrefundable and will be deducted off the consultation on the day as well as and a 12-hour cancellation policy. This appointment can go up to 90 minutes.

Acupuncture +/- Cupping Standard Appointment - $50.00

For existing patients only

Standard Acupuncture and or cupping appointment. Total consultation fee is $100 HICAPS and afterpay onsite.
12 hours cancellation, late reschedule and missed appointment fee of $50. Please contact the practice if you need to reschedule on 08 9505 8480.

Chinese Medicine I/C - $50.00

For new patients only

Chinese Medicine Initial Consultation. Parking can be difficulty please arrive 10 minute prior. We have $50 deposit which will be deducted on the day. We have a 12 hour cancellation policy if a late cancellation or missed appointment will result in the forfeit of the $50. Please call the practice on 08 9505 8480 to reschedule. Total cost of this consultation is $70 in which your deposit will be deducted from the total consultation price on the day of the appointment. Afterpay available.

Chinese Medicine Consultation Regular - $30.00

For existing patients only

Total cost of this consultation is $30 in which your deposit will be deducted from the total consultation price on the day of the appointment. Afterpay available. Please note we have a 12 hour cancellation/ reschedule or missed appointment fee in which your deposit will be forfeited.

Chinese Medicine Consultation Extended - $50.00

For new patients only

Total consultation fee is $100 and afterpay onsite. $50 Deposit required to book and secure the appointment which will be deducted from the cost on the appointment. We have a 12-hour cancellation/ missed or late cancellation policy- which is result in the deposit been forfeited.

Bowen Standard Consultation - $50.00

Bowen Therapy Massage.
Total consultation fee is $85 and afterpay onsite. $50 Deposit required to book and secure the appointment which will be deducted from the cost on the appointment. We have a 12-hour cancellation/ missed or late cancellation policy- which is result in the deposit been forfeited.
